Humana vision insurance plans and cost
Humana’s main individual vision plan is Humana Vision PLUS. Humana advertises coverage from $12 a month, based on one person in Maine; your price depends on your state and age. Humana’s fine print adds that a one-time, non-refundable enrollment fee may apply, that payment may include an administration fee, and that dental and vision plans may have a minimum one-year initial contract. Fees are disclosed when you enroll.
What you get, in network:
- Eye exam: $0 at a PLUS provider, $10 at other network providers
- Frames: $250 allowance at PLUS providers, $200 elsewhere, with 20% off the balance
- Standard lenses: $10 copay; standard progressives $65
- Contacts instead of glasses: $200 allowance
Humana also sells Humana Extend plans that bundle dental, vision and hearing benefits into one payment.
Humana's vision network: PLUS vs. regular providers
Humana Vision PLUS uses the Humana Insight network, which Humana’s plan summary identifies as the EyeMed Insight network. EyeMed is a third-party vendor; Humana and EyeMed are separate companies. The summary cites more than 214,000 access points, and Humana’s shopping page cites more than 225,000 in-network vision specialists.
Some providers are designated PLUS providers. They give you a $0 exam and a $250 frame allowance instead of $10 and $200, with no coupons or extra paperwork. Humana names LensCrafters, Pearle Vision and Target Optical among network retailers. Use the provider search on Humana’s site to see which offices near you are PLUS.
Out of network, you pay and are reimbursed up to set allowances: $30 for an exam, $200 for frames, $25 to $55 for lenses and $92 for contacts.
Humana vision insurance legitimacy and licensing
Yes. Humana’s individual dental and vision plans are insured or offered by Humana subsidiaries such as Humana Insurance Company, which insures the Minnesota Vision PLUS policy. Humana, Inc. is based in Louisville, Kentucky.
The BBB rates Humana, Inc. A+ and it has been accredited since September 1998, with 299 complaints in three years across all of its products. Trustpilot reviews average 1.4 out of 5 from 425 reviews, mostly about medical and Medicare coverage. The MyHumana app holds a 4.8 rating on the App Store.
What customers say about Humana
Recurring themes across the review sites and complaint records listed above, not individual quotes.
Common praise
- The MyHumana app is rated 4.8 out of 5 on the App Store
- Some reviewers credit individual Humana staff for sorting out billing problems
- $0 exams and a $250 frame allowance at PLUS providers
Common complaints
- Customer service that reviewers describe as unhelpful or hard to reach
- Denied claims and coverage disputes, mostly on medical and Medicare plans
- Premium increases and higher out-of-pocket costs than expected
- Providers leaving the network, and website or login problems
Humana vision insurance value for the price
At $12 a month, a year of Vision PLUS costs about $144 plus any enrollment fee. At a PLUS provider you get a free exam and $250 toward frames, so if you buy glasses each year the plan usually covers its cost. It is a weaker deal if you only need an exam, if no PLUS provider is near you, or if your state’s premium is well above the Maine example.
Vision insurance covers routine care. Eye injuries, infections and diseases like glaucoma go to your health insurance, so see a doctor right away for sudden vision changes.
Enrollment fee, contract and cancellation
Humana’s shopping page says a one-time enrollment fee may apply and is non-refundable as state rules allow, and that dental and vision plans may require a one-year initial contract. Humana does not publish the fee amount or its cancellation steps on its public vision pages, so ask an agent at 866-597-0895 before you buy:
- What is the enrollment fee in my state?
- Is there a minimum contract, and what happens if I cancel early?
- How do I cancel, and is any premium refunded?
If a dispute is not resolved, contact your state insurance department.
Complaints and what customers report
Most public complaints about Humana concern its health and Medicare plans. On Trustpilot, recurring themes are unhelpful or hard-to-reach customer service, denied claims, premium increases, providers leaving the network and website login problems; some reviews mention vision platform access issues. A smaller group of reviewers praise individual staff who resolved billing problems.
To avoid surprises with a vision plan, confirm the office is in the Humana Insight network (and whether it is a PLUS provider) before you book, and ask the store to verify your benefits before ordering glasses.
What Humana Vision PLUS does not cover
Humana’s plan summary lists exclusions including:
- Medical or surgical treatment of the eyes
- Vision training and orthoptics
- Two pairs of glasses in place of bifocals, trifocals or progressives
- Non-prescription lenses and safety eyewear an employer requires
- Orthokeratology and refitting after the initial contact lens fitting
- Lost or broken eyewear, except at normal benefit intervals
LASIK is not covered, but members can get 15% off retail or 5% off promotional prices through the U.S. Laser Network.

- VSP Individual Vision Plans Better if your eye doctor is a private practice in VSP's network or you shop at Walmart. Read review
- Humana Extend dental, vision and hearing bundle If you also want dental and hearing coverage, Humana's Extend plans combine all three in one monthly payment.
- Paying cash with HSA or FSA money If you only need a yearly exam, paying out of pocket avoids premiums, an enrollment fee and a one-year contract.
