UnitedHealthcare can be a good choice if its network has your doctors and the total yearly cost beats the alternatives in your county, but know your appeal rights before you need them. It is a legitimate, financially sound insurer and the largest in the country by medical members, with employer plans nationwide, marketplace plans in 30 states for 2026 and Medicare Advantage in 48 states plus Washington, D.C. Its Medicare Advantage plans averaged 4.11 stars in 2026, but analysts expect only about 67% of its members to be in 4 star or better plans for 2027. The tradeoff is service: claim denials, prior authorization and phone support dominate complaints, a federal criminal and civil investigation of its Medicare business is ongoing, and federal watchdogs flagged high denial rates for rehab and nursing facility stays.

Best for: People whose doctors and hospitals are in UnitedHealthcare's network, Medicare shoppers who want wide plan choice, and families or employers that need one insurer across many states.
Skip it if: Your doctors are outside its network, a lower-cost plan with similar coverage is sold in your county, or you expect to need rehab or nursing facility care and want fewer prior authorization hurdles.

UnitedHealthcare, step by step
Find out which door you qualify for
Most people get UnitedHealthcare through an employer, the ACA marketplace, Medicare or Medicaid. Each has its own enrollment window and rules.
Compare plans in your county
Use HealthCare.gov for marketplace plans, the Medicare Plan Finder for Medicare Advantage and Part D, or your employer's portal. Check that UnitedHealthcare sells in your county at all.
Check doctors, drugs and the yearly total
Look up your doctors and hospital in the plan's directory, confirm each prescription on the formulary, then add the premium, deductible and out-of-pocket maximum.
Enroll during the right window
Marketplace open enrollment starts November 1, 2026, and Medicare's Annual Enrollment Period runs October 15 to December 7, 2026.
Use the plan and keep records
Show your member ID card, ask whether a service needs prior authorization, and save every denial letter and Explanation of Benefits.
Appeal a denial if needed
File an internal appeal with UnitedHealthcare first. If it upholds the denial, you can ask for an independent external review (marketplace and employer plans) or move to the next Medicare appeal level.
UnitedHealthcare legitimacy and licensing
Yes. UnitedHealthcare is the health insurance business of UnitedHealth Group, a public company that files reports with the SEC. It covered 48.5 million people as of June 30, 2026, down from 50.1 million a year earlier, and AM Best rates its insurers A (Excellent), so it has the money to pay claims.
Legitimate is not the same as trouble-free. In July 2025 UnitedHealth Group said it had begun complying with formal criminal and civil requests from the Department of Justice about its Medicare business. The company says independent audits show its practices are accurate. A separate whistleblower case over Medicare Advantage risk adjustment, which the DOJ joined in 2017, is still unresolved. In February 2024 its Change Healthcare subsidiary suffered a ransomware attack that exposed data on an estimated 192.7 million people.
Leadership also changed. Andrew Witty stepped down as CEO on May 12, 2025, and board chair Stephen Hemsley took over. In December 2025 the company released outside reviews by FTI Consulting and Analysis Group of its coding, care management and prior authorization practices and said it would make close to two dozen changes.
UnitedHealthcare plans and what they cost
UnitedHealthcare sells almost every kind of health coverage, and there is no single price. What you pay depends on the plan type, your county, your age and any subsidy.
- Employer plans: about 29.9 million members. Your employer picks the plans and pays part of the premium.
- Individual and family plans: sold on the ACA marketplace in 30 states for 2026. KFF lists UnitedHealthcare as leaving Georgia’s marketplace for 2027. In its latest quarterly filing the company said it pledged to rebate 2026 profits on these plans to customers.
- Medicare Advantage and Part D: plans in 48 states plus D.C. In 2026 about 52% had a $0 premium and the average out-of-pocket maximum was $6,492.
- Medicare Supplement: UnitedHealthcare insures the AARP Medicare Supplement plans, with 4.26 million members.
- Medicaid: UnitedHealthcare Community Plan serves about 6.78 million Medicaid members in select states, plus dual plans for people with both Medicare and Medicaid.
- Short-term plans: limited coverage for gaps. These do not have to follow ACA rules and can exclude pre-existing conditions, so read the terms closely.
To price a plan, use HealthCare.gov or your state marketplace once 2027 plans post on November 1, or the Medicare Plan Finder for Medicare plans.
Is UnitedHealthcare good for Medicare?
UnitedHealthcare is the largest Medicare Advantage insurer, with 7.57 million members by its own count. Its 2026 plans averaged 4.11 CMS stars, higher than Anthem’s 3.63 but below Aetna’s 4.2 and Kaiser’s 4.41.
The 2027 ratings, released in October 2026, were weaker. J.P. Morgan analysts estimate about 67% of UnitedHealthcare members will be in 4 star or better plans for 2027, a double-digit drop, while CMS says about 71% of all Medicare Advantage drug plan enrollees are in 4 star or better plans. Star ratings vary by plan, so check the rating of the exact plan in your county.
Prior authorization is the biggest concern. Reports from the HHS Office of Inspector General, cited by Senators Blumenthal and Hawley in July 2026, found UnitedHealthcare denied 66% of requests to admit Medicare Advantage members to inpatient rehabilitation facilities, compared with an average of 41% at 16 smaller insurers, and that it reversed 99.7% of appealed denials for skilled nursing facility admissions. A class action filed in 2023 over its use of the naviHealth nH Predict tool for post-acute care is still in court; UnitedHealth denies the claims.
A free SHIP counselor can help you compare Medicare Advantage with Original Medicare plus a Medigap plan before the Annual Enrollment Period ends December 7, 2026.
What customers say about UnitedHealthcare
Recurring themes across the review sites and complaint records listed above, not individual quotes.
Common praise
- Large national network that works when members travel or move
- Well rated member app for ID cards, claims and finding care
- Individual agents who take time to explain benefits, once members reach them
- Many Medicare Advantage plans with a $0 premium and extra benefits
Common complaints
- Claim denials and bills that arrive after procedures members thought were covered
- Slow or denied prior authorizations for tests, surgery and rehab
- Long hold times and conflicting answers from customer service
- Rising premiums and out-of-pocket costs
- Denials that are later overturned only after the member or doctor appeals
UnitedHealthcare complaints and claim denials
The BBB lists 2,539 complaints against UnitedHealth Group in the last three years, and 91% of its 749 Trustpilot reviews are one star. The recurring themes are claim denials, slow or denied prior authorizations, unexpected bills, long hold times and conflicting answers from representatives.
To lower the odds of a denial and fix one faster:
- Before a planned test, surgery or facility stay, ask your doctor’s office whether UnitedHealthcare requires prior authorization and get the approval number.
- Confirm every provider is in network, including the anesthesiologist, lab and facility.
- Read each Explanation of Benefits. It shows the reason code for any denial.
- Keep notes of every call: date, representative name and reference number.
- Ask your doctor for a letter of medical necessity if a service is denied as not necessary.
You can compare complaint data for UnitedHealthcare’s insurance companies in your state with the NAIC’s Consumer Insurance Search, and file a complaint with your state insurance department if the company will not resolve a problem.
How to appeal a UnitedHealthcare denial
A denial is not the final word. Many denials are overturned on appeal, and federal law sets the deadlines.
Marketplace and most employer plans: file an internal appeal within 180 days of the denial notice. UnitedHealthcare must decide within 15 days for care you have not received yet, 30 days for care already received, and no more than 4 business days for urgent care. If it upholds the denial, you can request an external review within 4 months. An independent reviewer decides, usually within 45 days or 72 hours if urgent, and the insurer must follow the decision.
Medicare Advantage: ask the plan for a reconsideration within 65 days of the denial notice. The plan has 30 days for a service you have not received, 60 days for a payment, and 72 hours for a fast appeal if waiting could seriously harm your health. If the plan says no, your case goes to an Independent Review Entity, and there are five appeal levels in all.
Medicaid: follow the appeal and state fair hearing instructions in your denial letter. Your state Medicaid agency can explain the deadlines.
Your state insurance department, a SHIP counselor (for Medicare) or a consumer assistance program can help you file at no cost.
How to switch or cancel UnitedHealthcare
When you can leave depends on how you got the plan. Marketplace plans can be changed during open enrollment, which starts November 1, 2026, or after a qualifying life event such as a move or job loss. Employer plans change at your employer’s annual enrollment or after a qualifying event. Medicare Advantage and Part D plans can be changed from October 15 to December 7, and Medicare Advantage members also have the January 1 to March 31 window to switch plans or return to Original Medicare.
If UnitedHealthcare is leaving your market, as in Georgia’s marketplace for 2027, you will need to pick a new plan during open enrollment or you could lose coverage. Always enroll in the new plan before dropping the old one to avoid a gap.
Who should pick something else
If your doctors or local hospital are not in UnitedHealthcare’s network, its size does not help you. In a Kaiser Permanente area, Kaiser often rates higher on quality. For Medicare, Humana improved sharply in the 2027 star ratings, and Original Medicare with a Medigap plan avoids most prior authorization rules. If you buy your own coverage, compare every insurer on HealthCare.gov with tax credits applied rather than starting from the brand.
Alternatives to UnitedHealthcare
- Kaiser Permanente Better if you live in a Kaiser service area and are fine using its own doctors and hospitals; it held 8 of NCQA's 18 five-star ratings in 2026. Read review
- Blue Cross Blue Shield Covers 118 million people through 33 local companies in every state; prices, networks and service depend on your state's Blue company. Read review
- Original Medicare with a Medigap plan The public option for people 65 and older who want any doctor that takes Medicare and no prior authorization network rules, at the cost of a higher monthly premium.
- HealthCare.gov marketplace or Medicaid If you buy your own coverage, compare every insurer in your county with tax credits applied, and check Medicaid if your income is low.

Frequently asked questions
Is UnitedHealthcare a good insurance company?
It is financially strong and has one of the widest networks, and its Medicare Advantage plans averaged 4.11 stars in 2026. Member reviews are poor, though, with a 1.2 Trustpilot score and 2,539 BBB complaints in three years, mostly about denials and service. Whether it is good for you depends on the plan, its network and the cost in your county.
Is UnitedHealthcare the largest health insurer?
Yes. UnitedHealthcare covered 48.5 million medical members as of June 30, 2026, including 7.57 million in Medicare Advantage, by its own count.
Why does UnitedHealthcare deny so many claims?
UnitedHealthcare does not publish an overall denial rate. Federal inspector general data showed it denied 66% of Medicare Advantage requests for inpatient rehab admissions, and senators have pressed the company on its prior authorization practices. Common reasons include missing prior authorization, out-of-network care and medical necessity decisions, all of which you can appeal.
How do I appeal a UnitedHealthcare denial?
For marketplace and most employer plans, file an internal appeal within 180 days, then request an independent external review within 4 months if it is upheld. For Medicare Advantage, request a reconsideration within 65 days. Your denial letter lists the exact steps.
Is UnitedHealthcare under investigation?
Yes. In July 2025 UnitedHealth Group said it was complying with formal criminal and civil requests from the Department of Justice about its Medicare business. The company says its practices are accurate, and we found no announced resolution as of October 2026.
Was my data exposed in the Change Healthcare hack?
Possibly. The February 2024 ransomware attack on UnitedHealth's Change Healthcare unit affected an estimated 192.7 million people, including many who were never UnitedHealthcare members. Watch for a notification letter and review your insurance statements for services you did not receive.
Does UnitedHealthcare sell marketplace (Obamacare) plans?
Yes, in 30 states for 2026. KFF lists it as leaving Georgia's marketplace for 2027, so check HealthCare.gov or your state marketplace when 2027 plans post on November 1.
Is UnitedHealthcare good for Medicare Advantage?
It offers plans in 48 states plus D.C. with a 4.11 average star rating in 2026, but analysts estimate only about 67% of its members are in 4 star or better plans for 2027. Compare the exact plan's rating, drug costs and doctors in the Medicare Plan Finder.
Is AARP Medicare Supplement the same as UnitedHealthcare?
AARP Medicare Supplement plans are insured by UnitedHealthcare, and AARP endorses them. They are Medigap plans that work with Original Medicare, not Medicare Advantage plans.
Does UnitedHealthcare offer Medicaid?
Yes, through UnitedHealthcare Community Plan in select states, with about 6.78 million Medicaid members as of June 2026. Eligibility and enrollment go through your state Medicaid agency.
